About the role
An exciting professional development opportunity exists for a motivated and passionate nurse within the Kids GPS (Guided Personalised Service) Care Coordination team. The position will be a 12 month maternity relief backfill contract.
The successful applicant will work as a Clinical Nurse Specialist (CNS Grade 2) and be responsible for coordinating, monitoring and evaluating the provision of care in order to streamline services and reduce the amount of time spent in hospital for patients with chronic and complex conditions.
The role will be based at Sydney Children's Hospital Randwick and forms part of the wider network service. The CNS2 will work in collaboration with patients and families, community clinicians/stakeholders, and SCHN multidisciplinary teams to deliver an effective and efficient care coordination service for chronic and complex patients.
This position is temporary part-time, Wednesday - Friday (0.58 FTE) up until 11/05/2025.
